Our Care Connected (Part of the Sussex Health and Care Partnership Digital Programme) gives health and care professionals and local people in Sussex shared online access to records and data through three digital enabler projects:

- Plexus, an e-record platform for GPs, hospital and community teams
- My Health and Care Record, to help people stay well and manage a condition or treatment
- Sussex Integrated Dataset, used to analyse how local health and care services can be better designed and run

Our Care Connected supports the public and practitioners as expert partners in the development and co-design of digital health and care services in Sussex.

Working in the Our Care Connected team offers a genuinely exciting opportunity to help develop the use of Agile practices in the Sussex Health and Care system.

This is an exciting opportunity to make a real difference as a senior manager within the Sussex Health and Care digital enabler programme, Our Care Connected (OCC). You will be responsible for management of the delivery team function, Delivery/Value Stream Managers, their teams, and leading the development of the new Engagement team.

Important to this role is effective stakeholder management and relationship building coupled with planning, delivery, reporting and monitoring of technical products and services built and developed by the technical development team.

The programme is dynamic with potential for changing priorities and therefore the ability to work flexibly to respond to changes is important.

**Key responsibilities include**:

- You will be working with the Programme Director to ensure the Domains and Value Streams continue to develop to complement the programme governance and that they actively contribute and inform the future development of priorities for the programme.
- You will manage the Delivery Team, leading the various delivery team meetings and line managing Delivery Managers and the new Engagement team, working closely with the Capability Leads and the reporting as required to a variety of management boards.
- You will keep the structure of the Delivery team function under review and make recommendations to ensure it remains fit for purpose in a changing environment.
- You will refine and update meeting processes as necessary to ensure business continuity of projects and the ability to report and action on any blockers or issues.
- The value stream work is developing within the programme, and this role will be responsible for the further development of the value streams.
- This role will lead the development of a new Engagement function that ensures the work undertaken by the OCC programme is understood by stakeholders. This will also require liaison with the ICB Communications team and working across the delivery portfolios
- The role also includes directly managing a portfolio which currently is Social Care work, liaising with the three Local Authorities and Sussex wide Registered Care Associations and NHSE Digital Social Care (DiSC)
- Sussex is also part of the National Front Runner Discharge Programme and as such is developing new ways to utilise the flow of digital data to improve the discharge data flows, reduce delays, and increase the efficiency of patient discharge across the Sussex care system as a whole. This role is very much part of ensuring delivery of the priorities from the Front Runner programme
- You will have the opportunity to hold a key role in driving user-centric implementation of existing transformation plans, as well as continual improvement working in partnership with health and care organisations across the Sussex Integrated Care System (ICS).

**As a Delivery Manager and Value Stream Lead, you will**:

- Provide an important part of the programmes engagement with partner organisations and key stakeholders
- Lead the Value Steams further development in association with the Programme Director
- Manage the Delivery team function and ensure processes for documentation are consistent across the delivery/value stream function
- Motivate/coach teams, making sure there is an iterative 'build and deploy' plan ensuring there are clear processes being followed for each of the projects and the is a standardised approach being developed
- Encourage teams to collaborate, communicate and focus on what is most important
- Proactively manage dependencies, overcome obstacles, and get the best value within constraints
- Write business plans and as necessary liaise with finance/procurement to ensure appropriate funding/payment processes are in place
