Assistant Practitioner

To assist Occupational Therapist, Physiotherapist and Community Nurse working under supervision in providing an Intermediate Care Service to patients to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge and maximise independent living within the recommended time frame of six to eight weeks.
The main duties of this job is to make a difference by helping patients through recovery and returning back to health and independence through rehabilitation. The majority of the work will be completed within the unit environment, however may include working within the community on occasions. This will include occasional weekend working Baywide.
The new team will be multidisciplinary and will have a home first ethos working closely with the GP and community nursing services.The team will be based in a 24 bed rehabilitation unit. This will be a supportive and professionally stimulating environment and an exciting opportunity for the new post holders.
