**Job Purpose**

To provide an integrated one team approach to Family Support Outreach Services to children, young people and families across the Bensham & Lobley Hill, locality and surrounding areas. To plan, deliver and assess outcome focussed work with the aim of providing holistic needs led support including outreach, projects, programmes, activities and services. To assist and advise families experiencing short
- or long-term challenges. Primary duties include assessing the needs of clients, providing support to and empowering them to overcome the difficulties they are experiencing.

**Main Accountabilities**
- To deliver an integrated Family Support Outreach Service across the 0-19 age range. Ensuring this is delivered throughout by supporting children, young people and their families’ - giving them opportunity to access a broad reach of holistic & community-based interventions.
- To be committed to providing an integrated one team approach to children, young people & their families across the 0-19 age range with an aim to delivering effective services and interventions.
- To have an understanding and to comply with, local and organisational procedures for promoting and safeguarding the welfare of children, young people and vulnerable adults.
- To work together with other organisations and agencies in order to promote safe working practices for children and young people in line with the Children’s Act 2004 - this will include liaising & referring into Children & Adults Social Care.
- To maintain accurate case records and up-to-date case files. To be confident with case management and understand the need to monitor outcomes and trends to ensure the effective delivery of services on a continuing basis.
- To provide direct support and guidance to families in their homes or in other community settings in relation to the health, care and well-being of children and young people across the 0-19 age range with an emphasis upon promoting children & young people’s communication, language, physical, social and emotional development.
- To develop your knowledge of local resources, including community and statutory services such as: Health, Education, Children & Adults Services alongside other voluntary & community sector services, whilst sign-posting and directing service users to these where & when appropriate.
- To attend team meetings on a regular basis, to make a commitment to regular supervision, to be responsive to critical challenge, advice, feedback and direction, and to demonstrate a commitment to training and development
- To represent St Chads CP core values and behaviours and ensure that the work you are responsible for is in line with St Chads CP policies and procedures.
- To comply with St Chads CP Diversity & Equality Policy in every aspect of your work and positively promote the principles of these policies amongst colleagues, service users and other members of the community.
- To comply with St Chads CP Health and Safety Policy, Data Protection Policy and to protect your own and others’ health, safety and welfare.
- To work flexibly as may be required by the needs of the service and carry out any other reasonable duties as required.
- To contribute towards St Chads fundraising and grant seeking activities.
- Working in partnership with other health, education and social care professionals to promote the family’s wellbeing according to agreed outcomes.
- To successfully deliver Family Support sessions and groups. To utilise the information gained when leading these groups to recognise, plan and deliver future sessions that will benefit the community.
