This is an exciting new opportunity to be part of an innovative new service optimising resources within Derby City, reducing demand and releasing capacity and more importantly empowering patients to self-care and promote their independence.

The Self-Management facilitators team will provide a programme of interventions in the Derby City area.

The overall aim of the service is to work with individuals to increase their knowledge, skills and confidence to manage their long-term conditions and care needs.

This is a clinical role and will involve care delivery with a behavioural change focus.

You will work closely with individuals to empower independence with their health and social care needs and use the Goal setting approach to help achieve this.

You will continue to promote patient centred care and utilise the shared decision making tools.

This job allows for innovative thinking and collaboration with other providers to support individuals with a sustainable change that means the most to them.
