We have a fabulous job opportunity for you to join our awesome team as a Support Coordinator. We support our brilliant customers who have enduring Mental Health diagnoses.

They live in a hospital step-down service in Levenshulme where, over a period of 12 weeks we support our customers to bridge the gap between discharge from a Psychiatric ward to the next step of their life.

You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. As our Support Coordinator, you’ll create bespoke and personalised support plans with and for your customers. Each plan will look to meet our customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.

You’ll also have responsibility for coordinating our support workers, apprentices and volunteers, and working collaboratively with external agencies, to assess and engage support for our customers.

**About you**

You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe) You’ll be given the latest kit, which could include a Surface Pro tablet and smart phone, supported by our new online integrated IT system. This means that you don’t have to be in the office as you can complete support plans on the go.

To make sure our customers are well supported, it’s important that you’ve had a job in a supported housing or mental health rehab environment before. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group
- When you start your job, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one._

**Working hours**

You’ll work on a rota basis between the hours of 7:00am and 10:00pm and will take your turn working days, evenings, weekends and bank holidays. We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you

Our manager is Liam and there are 10 of us in the team. We don’t just love to support our customers, we’re there for each other too
