Hospital at Home Community Matron

The post-holder will be an excellent communicator and work well both individually and within a team. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter-professional team.
To work as part of the Integrated Community Care team to prevent inappropriate hospital admission and have the capability to provide assessment and treatment of patients.
To deliver a high standard of patient care using evidence based framework within East &North Hertfordshire.
The post holder will work autonomously to provide a comprehensive,
advanced assessment and care package to the patients and families, utilizing support services and liaising with other agencies to ensure the complex needs of this client group are met.
To provide advanced nursing assessment, diagnostic, prescribing, care and treatment, marshalling resources, overseeing the quality of care from all agencies and co-coordinating acute, primary and social care ensuring that the focus of care remains at home and in the community where possible.

This post carries continuing responsibility for pro-active management of a caseload consisting of identified
high intensity users with complex needs. The post holder will work autonomously to provide a comprehensive,

focus of care remains at home and in the community where possible.
