- **About the role**
An exciting opportunity has arisen for a permanent post as Assessment and Engagement Coordinator to support Humankind’s service delivery within Reconnected to Health in HMP Durham.
- As an Assessment and Engagement Coordinator you will work as part of a team to carry out assessments and deliver drug and alcohol recovery interventions to enable individual service user progression from engagement, through structured treatment to sustained recovery.
- You will manage an agreed service user caseload, reviewing on-going care and treatment and liaising closely with the full range of partners, as well as family members and significant others, to achieve and sustain recovery goals.
