As the Care Coordinator, you will be responsible for the overall management and scheduling of client rotas and will participate in the on-call rota & Monitoring. With outstanding planning and organisational skills, you will ensure care resources are used efficiently, using excellent communication skills throughout all call planning and delivery. You will have a positive attitude and lots of energy, using innovation in your problem-solving. Main Responsibilities: - Client Assessment & Care Planning: Conduct initial assessments to understand the care needs of potential clients. Develop, review, and update personalized care plans in collaboration with clients, their families, and healthcare professionals. - Coordination & Scheduling (ROTA): Coordinate care services by matching the right carer with clients based on needs, preferences, and skills. Ensure timely scheduling and adjust as necessary due to changes in client needs or caregiver availability. Ensure weekly rota scheduling - Conducting Supervisions & Spots checks Ensure monthly spots check & Supervisions are in place and regularly reviewed Lias with sidra to check matrix. - Weekly on call monitoring Check staff clocking in & out, carer notes, medications or complaints. - Communication & Relationships: Serve as the primary point of contact for clients and their families, addressing any concerns or changes in care requirements by creating reports. Foster strong relationships with clients, families, and caregivers to ensure a smooth care delivery process. - Training & Development: Collaborate with the training team to ensure caregivers are equipped with the necessary skills and knowledge. - Identify areas of improvement and recommend training or mentorship opportunities. - Conduct refreshers training with staff lias with Annus - Quality Assurance & Compliance: Regularly review and audit care logs, reports, and feedback to ensure the highest quality of care. - Ensure compliance with all regulatory standards and company policies. Team Collaboration: - Work closely with other team members, including Annus and Nikki and Sidra, to ensure seamless operations. - Participate in regular team meetings, providing updates and insights on client care. - Assist in the completion of audits and subsequent action plans to improve business processes - Support the Registered Manager - Seize opportunities to assist in other areas of the business that match your skillset, such as completing assessments or representing the company at external events, Spots check, Supervisions, - Managing PPE & uniform for staff Maintain PPE for staff and ensure staff have full PPE & uniforms. Order PPE or uniform in good time. - Managing Shadowing for new Staff Lias with senior carer and send new staff on 2 days shadow or complete shadowing form and assessments

**Salary**: £22,000.00-£24,000.00 per year
