An exciting job opportunity has arisen in our Community COPD and Home Oxygen Service and we are looking for a physiotherapist or nurse with respiratory experience from previous acute, sub-acute or community roles to join our innovative team.

The overall service aims to improve patient quality of life, to improve the safe use of oxygen and to support patients to self-manage their disease helping to reduce emergency admissions to hospital. We collaborate closely with all stakeholders to provide an integrated approach to patient care across the system and there is a significant educational component to this role.

The main role will be co-leading the Home Oxygen Service. However, the team works flexibly with an expectation that all pathways may need to be covered with the correct support and training (e.g. Domiciliary Visits for COPD management, Spirometry for diagnosis and Pulmonary Rehabilitation). You should have a broad knowledge base with respiratory patients, including the assessment and treatment of people with COPD and those that have home oxygen. It would be beneficial to have previous experience and skills in the delivery of spirometry and Pulmonary Rehabilitation.

The team is based at St Pancras Hospital and operates 9am - 5pm Monday to Friday. The post-holder will be required to contribute to a rotating on-call telephone advisory service which runs from 8-9am and 5-8pm, Monday to Friday.

The team is comprised of Respiratory Nurses, Physiotherapists, Rehabilitation Assistants, Psychologists, Respiratory Physicians and a Smoking Cessation and Behaviour Change Specialist. It has a strong culture of integrated working between community and secondary care, established through close communication, weekly MDT’s, joint training and clinical support from Respiratory Consultants at the Royal Free Hospital and UCLH. There is also a strong emphasis on learning in an open and safe environment where staff are empowered to see situations as opportunities to grow and enabled to increase their knowledge and skills.

CNWL provides NHS services throughout a person’s life, in physical and mental health and everything in between, at GPs and hospitals to the community and in their own home. The majority of our services are provided in the community, which means treating people in their homes or from clinics close to home. In this sense we are a very modern part of the NHS, something the new NHS Long-Term Plan wants to see more of, with better integration and coordination of care, delivered as close to home as possible.

**The main clinical responsibilities include**:

- Responsibility for the smooth running of the HOSAR pathway on a day to day basis.
- Report to the Clinical Lead and Manager and team as required.
- Develop and implement Camden HOS-AR protocols
- Work with all stakeholders to promote safe and effective oxygen prescribing within guidance.
- To represent Camden at relevant stakeholder meetings.
- Oxygen assessment and review in clinic setting and patients home, including taking capillary blood gases
- All types of oxygen therapy assessments and reviews including ambulatory oxygen assessments.
- Organise and provide service specific in-service training (IST) and training to others as required.
- To communicate and present cases at the weekly team caseload and/or MDTs and support less experienced staff members with this process.
- To collect HOSAR data and report as required.
- To ensure that high standards of clinical practice and patient care are maintained and to ensure that other clinicians in the team do likewise.
- Triage and assess referrals into the team, including gathering medical history from GP records/referrer and identifying if appropriate or not and feeding back relevant information as required.
