The role of a Mental Health Practitioner is within the Community Older Persons Team. The team are well established, supportive of one another, where the wellbeing of each other and our client group is at the forefront of all we do.We are fortunate in our location, enjoying views and access to the sea front whilst being in the centre of the area we serve.

The role involves managing a defined caseload both organic and functional in nature. The focus is on providing continuous assessment and treatment to patients and their carers. The liaison with a wide range of statutory and non-statutory agencies is central to this role. The post holder will be an effective member of the multi-disciplinary team.
