Social prescribing link workers will help to reduce health inequalities by supporting patients unpick complex issues affecting their wellbeing.

The role of social prescribing within the practice will be a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services

Work with a cohort of people to support their personalised care requirements, using the available decision support aids.

Help people to manage their needs, answering their queries and supporting them to make appointments access support services

Support people to take up training and employment, and to access appropriate benefits where eligible.

Ensure that people have good quality information to help them make choices about their care,

Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.

Provide coordination and navigation for people and their carers across health and care services, alongside working closely with practice based social prescribing link workers, and other primary care roles.

Support the coordination and delivery of MDTs

Build relations with key staff in general practice within the local Primary Care Network, attending relevant meetings, becoming part of a wider network team, giving information and feedback on social prescribing.

**Job Type**: Part-time
Part-time hours: 25 per week

**Salary**: From £11.00 per hour

**Education**:

- Certificate of Higher Education (preferred)

**Experience**:

- Social presciribing: 2 years (preferred)
- Health and Social Care: 2 years (required)

Licence/Certification:

- NMC (preferred)
- Driving Licence (preferred)
