Manage vaccine clinics with the support of the trust divisional vaccination group, ensure that all eventualities are planned for within local based clinics and the service offered is an efficient and person centred experience
- Support the locality delivery of vaccinations and support service improvement methodologies, evidence based practice and change management
- Ensure all staff in the vaccination clinic hubs are inducted appropriately and support locality vaccination champions
- Lead on ways to facilitate and improve vaccine uptake
- To develop and maintain relationships across clinical, professional, and non-clinical services to ensure the smooth delivery of the trust vaccination programme.
- To deliver training in partnership with the trust clinical skills facilitators to ensure the vaccination workforce are provided with the correct skills and knowledge.

- Demonstrate highly developed, effective and evidence-based nursing care and act as clinical advisor and educator to junior staff and overall responsibility for clinical care to administer vaccines safely and in line with best practice guidance
- Maintain personal contact with patients, relatives and carers, being sensitive to their need for courtesy, dignity and privacy
- To identify, prioritise and implement programmes of care that meet the needs of the clients
- To take into account risk and its effective management e.g. consent, side effects
- To encourage participation of clients, carers and relatives in assessments and in care provision pre and post vaccines administration
- To refer to other practitioners when clients’ needs are beyond their own role or scope of practice
- To identify and record all new problems and concerns and report back to the Deputy Director of Nursing
- To undertake health promotion and disease prevention
- To provide information and support to promote and optimise positive health
- To provide care and attention to the needs of vulnerable older adults including risk assessments and taking appropriate action as required
- Where incidents are reported, initiating appropriate action and keep the Head of Service informed of problems and progress
- Lead on infection control and prevention initiatives ensuring all relevant audits are completed and action plans implemented to improve clinical practice
- Lead regular caseload profiling and contribute to community health needs assessment, agreeing health improvement priorities and specific contribution of the community adult health service in collaboration with GP practices, nursing/therapy and AHP colleagues. Set and monitor objectives and targets for team activities with team leaders and others.
