This is an integral part of the out of hospital care model designed to ensure that patients receive appropriate care and treatment as close to home as possible. The post holder will work autonomously, as part of Responsive Services responsible for assessing, diagnosing, treating, and evaluating care for adults requiring complex management with the aim of preventing inappropriate hospital admission and facilitating timely discharge. Working closely with patients and relatives, GPs, secamb, social services and the third sector to develop crisis management plans and support avoidable hospital admission. The post holder will demonstrate safe, competent clinical decision making and expert patient care including diagnostic skills.

They will communicate and work collaboratively with the MDT to meet the needs of patients, supporting the delivery of policy and procedures. The service operates 8am to 8pm seven days a week Please see job description and person specification document attached for full details.
