An exciting opportunity has arisen for a Care Coordinator to join the Macclesfield Primary Care Network as part of our multi-disciplinary team (MDT), working across all six GP Practices.

As a Care Coordinator you will be critical in supporting the work of the MDTs, coordinating the care of an identified caseload of patients, and liaising between the professionals and services involved, and the patients and their families.

This will be a key role within our Personalised Care Team.

**Main duties of the job**

As a Care Coordinator, you will:

- To be responsible for the organisation of MDT meetings; supporting the coordination and delivery of MDTs within the PCN.
- Co-ordinate care packages for patients as identified by the GP across health, social care and mental health as appropriate, providing a single-point of access for staff & service users, actively managing patients care plan delivery.
- Facilitate smooth and planned discharge and handover between care settings across the health and social care system, ensuring information is effectively shared between settings including general practice, hospitals, care homes and care agencies, and be responsible for facilitating inter-agency communication and support.
- Assist in coordinating patient care in different areas such as mental health, frailty, cancer, pediatrics, long term conditions etc
- Identify and work with a list of named patients with the aim of encouraging independence, enabling people to remain at home, reducing unnecessary admissions to hospitals and supporting discharge from hospital, improving the quality of care.
- Provide coordination and navigation for patients and their carers across health and care services, working closely with Social Prescribing Link Workers and other Primary Care roles.

**Desirable**
- Knowledge of clinical systems (Emis)

- Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
- Experience of data collection and providing monitoring information to assess the impact of services
- Experience in delivering training to user groups / staff teams
