An opportunity for 2 Registered Nurses / Allied Health Professionals to join Pilgrim Hospital Discharge HUB in its mission, to support the wards in delivering timely and effective discharge. To provide direction to members of the MDT and support services (e.g. pharmacy) during board round to ensure achievement of transfer of care

To have an in-depth knowledge of all aspects of care throughout the patient journey and to communicate in an appropriate and sensitive manner highly complex information to patients, families or carers ensuring that barriers to communication such as language difficulties, hearing impairments or cognition issues are addressed.

To take a proactive approach to the discharge management of patients with complex needs. This will involve communicating with Family/Carers, Social Service, Continuing Healthcare, Community Services, GP’s, Care Homes and other service providers to ensure any health or social care issues that may impact on a timely transfer of care are identified and resolved early in the patient pathway.
