An exciting opportunity has arisen for a dietitian to work in our community prescribing support service on a fixed term basis to cover maternity leave.

This exciting post involves provision of a service to Nursing /Residential homes and the housebound, within our 5 Care Communities across the East of Cheshire Clinical Commissioning Group. This will include individual, holistic patient care and delivery of training for Nursing and care staff, and other Health Care professionals. You will be working within the wider MDT, demonstrating excellent clinical outcomes and quality of care.

We are looking for a forward thinking, motivated dietitian who can demonstrate varied experience of different clinical conditions, good communication and organization skills.

The Dietetics service is committed to service improvement and providing support in the role through Continuous Professional development, appraisal, clinical supervision and mobile working.

**CLINICAL**:
1. To practice safely within the professional code of conduct and level of competency for all aspects of work, including the management of patients in your care
2. To deliver the dietetic service to a caseload of patients in residential/ nursing homes and housebound patients which requires specialised nutritional knowledge such as, enteral tube feeding, elderly medicine, stroke management, diabetes, weight management, oncology, IBD, IBS and degenerative disorders. This includes managing and prioritising a defined patient caseload within the community setting.
3. To accept referrals of patients discharged from hospital on nutritional supplements and directly referred from GP’s and the community medicines management team, as per the agreed referral pathway.

**PROFESSIONAL**:
1. To initiate, plan, deliver and evaluate training for nursing home staff, carers and other community health care professional
2. To promote the department Clinical Governance agenda by continually monitoring the outcomes of care and cost effectiveness of dietetic intervention in this patient group
3. To monitor statistical data and outcomes of dietetic interventions regularly and share as appropriate at relevant joint dietetic/medicines management meetings, Nursing home and GP support meetings and utilise for service improvement.

**LEADERSHIP**:
1. To lead audit within designated GP practices on cost effective prescribing of oral nutritional supplements in GP practices across east Cheshire Clinical commissioning group. This will involve working closely with the medicines management team and other band 6 prescribing support dietitians
2. To supervise and provide clinical support to the dietetic assistant, through case supervision, development and monitoring of competencies and providing guidance on audit projects.
