We are looking for experienced Complex Care Support Workers on behalf of Yorkshire Community Healthcare.

We are looking for both day and night support workers.

The role involves supporting a client both in their own home and out in the community, as required.

Working on a one to one or two to one basis, your role will include:

- working to a person centred support plan
- keeping accurate and up to date records and daily notes
- supporting client(s) with personal care and moving and handling
- liaising with other health professionals
- building strong and effective working relationships with clients and colleagues

We are looking for people who have:

- previous experience supporting clients with complex health needs including tracheostomy, PEG feed, catheter or stoma
- sound knowledge and experience of supporting clients with moving and handling and personal care
- patience and empathy
- an ability to build positive relationships
- a commitment to providing the highest quality care and support
