Main tasksa) Plan and organise the commissioned Type 2 Education (T2ED) programme in accordance with key performance indicators and quality standards. Liaise with clinical team to adjust content of programme as required.b) Develop health promotional material to engage and encourage attendance by target population(s) including active promotion of the T2ED events to relevant audiences by a variety of methods. Maintain library of national materials appropriate for use.c) Be open to feedback from attendees and non-attendees to encourageincreased participation in T2ED programmes and events. Engage with clinical / administrative staff and other healthcare professionals in external agencies to assist with promotion of T2ED.

Provide training as required to encourage attendance.d) Collate and analyse performance information to better understand the needs of the population(s) and encourage uptake of education programmes by those who are clinically appropriate. Capture patient feedback with integrity and respect for confidentiality. Present performance information to leadership team and make recommendation where appropriate. Main responsibilitiesa) Plan and organise the provision of T2ED across venues, in accordance with staffing rotas and patient need, implement adjustments when necessary.

b) Analyse T2ED attendance data with a view to improving numbers who attend. Compare a range of demographics and patient information in order to identify and remove potential obstacles to meeting key performance indicators, eg difference in attendance by population(s) with type 2 Diabetes. Review referral and booking procedures and make recommendations with regard to processes in order to improve attendance and patient engagement. c) Work autonomously while collaborating with colleagues in the multi-disciplinary teams in order to provide an effective and high-quality service.

Provide updates on the attendance by patients to T2ED to the leadership team and make recommendation to improve attendance outcomes, which may include recommendations beyond own working practice by others in the team. This does not include responsibility for implementing changes to working practices beyond own are of work. d) Present activity and outcome measures to evidence the importance of the project to all staff, including those from external organisations. Regularly attend team meetings.

e) Lead communications in order to promote the service in a timely manner, working with administrative colleagues as required. Make regular contact with patients to understand reasons for non-attendance adhering to quality improvement principles. Undertake surveys as required in accordance with SCFT policies regarding data protection.f) Liaise with the Operational Lead and Clinical Service Manager around budget and costs associated with delivering T2ED, making recommendations for expenditure and savings as required.g) Support Trust Information Governance service requirements and ensure all information is handled in accordance with good practice and shared appropriately.
