East Cheshire NHS Trust are looking to recruit enthusiastic and highly motivated Advanced Care Practitioners (ACP's). You will be joining a well established and passionate team of clinical staff who put patients at the heart of what they do.

The ACP's will provide clinical leadership for the integrated team in particular the Urgent Crisis Response and Specialist Virtual wards (Frialty and Respiratory) enabling people to remain in their own homes and prevent avoidable Accident and Emergency attendances to support adults with a health/social crisis at home. The service also supports the Discharge 2 Assess initiative to provide timely assessment for rapid hospital discharge.

You will work strategically, contributing to the planning and development of integrated services, acting as a driver for change and ensuring standards of care and best practice in line with up to date evidence base.

If you are an experienced ACP and successfully completed the transition to advanced practice module and either has or are working towards their Masters’ award, then this is an opportunity for you.

To undertake advanced clinical assessment of patients with differential and undiagnosed illness or exacerbation of existing conditions using advanced clinical skills, critical thinking, decision making and risk analysis to provide safe, high quality, evidence based care preventing unnecessary acute hospital admissions, supporting patients in their own homes for longer and preventing premature admissions into long term care facilities

To provide proactive caseload management and acute intensive support/ intervention to manage clinical episodes/care in partnership with patients, families and other health and social care professionals

Provision of advanced practice support to members of the multi-professional team through provision of professional advice, consultancy, clinical expertise and leadership to patients, carers and staff. This will be done in collaboration with senior colleagues within the team and will require demonstration of advanced skills and competencies to influence the delivery of programmes of care in the management of patients
