We are looking to recruit a full time, Risk Assessor to deliver a high-quality, person-centred risk assessment in the Hammersmith, Shepherd's Bush, Fulham and Ladbroke grove area.

**The Role**:

- Carry out new assessments daily and create care plans for new service users within our areas
- Liaise with the team regarding any changes to service users reviews
- Support service users to achieve maximum independence
- Ensure adequate knowledge and comply with risk assessments
- Record complaints, incidents, accidents as per the reporting policy, keeping all parties involved up to date
- Updating existing care plans and reviews.

**Experience**:

- Providing care: 1 year (preferred)
- Supervisory: 1 year (preferred)
- Assessor 1 year (preferred)
- Training available, if no experience.

**Work remotely**:

- No

**Job Types**: Full-time, Permanent

**Salary**: Up to £21,000.00 per year

Schedule:

- Monday to Friday
