Due to promotion, the Dementia and Delirium Outreach Team have a vacancy for a Dementia Education Facilitator. We are looking for an individual to join our motivated team, to continue the formulation of a tiered education package and support its delivery.

The dementia and delirium outreach team support patients and their families in the acute hospital and community hospitals who are living with a dementia or experiencing a delirium, as well as increasing staff confidence and knowledge whilst caring for people in this patient group.

This role requires an individual to continue the work of writing new and innovative ways of delivering a tiered dementia training package as well as supporting delivery of this across the organisation. This role also involves supporting staff working with patients with dementia and delirium within both inpatient wards and community teams across Somerset, by advising on approach and delivery of care to increase staff’s knowledge and confidence.

The post holder will work with the Dementia and Delirium Specialist Nurse, senior managers, the directorate practice facilitators and learning and development to identify core clinical skills for dementia and delirium, to continue to develop and deliver tiered training to individuals, groups or teams providing care to patients with these diagnoses. The role will work alongside the existing clinical skills team to ensure the focus for the directorate is in keeping with the clinical skills plan of the organisation.

The post holder will further oversee the Dementia Champions across the organisation and will advise on approach and delivery of care to people with dementia and delirium across the Trust and will also develop and maintain data collection and evaluation tools for training delivered.
