Physical health outcomes in people diagnosed with serious mental illness are significantly poorer than in the main population. In order to address this inequality this role will support service users across our community mental health teams to undertake annual health checks and should be offered proactive advice on how they can make lifestyle changes and improve their physical health.

Working across a range of community mental health services the post holder will work closely with the teams undertaking annual physical health checks, in a clinic environment and, where this is not possible, in the Service users’ own home.

The role is Locality based, covering geographical areas in a mixture of clinic-based and home visits. The Patient cohort is from the Recovery, Assertive Outreach and Later Life mental health caseloads.

The post holder will be required to work independently through an allocated workload with guidance and direction from senior staff.
