The Paediatric Neurodisability Nursing Team provides co-ordination and case management of children with highly complex and unstable long-term conditions in partnership with health, education and social care organisations.

The aim of the service is to provide a dedicated specialist nurses to promote standards of excellence and manage the clinical risk of complex, unstable and vulnerable children in the community within the context of family centred care thereby reducing the need for secondary care interventions.

The CNS for challenging behaviour and learning disabilities will support CYP with challenging behaviours related to ASD, sleep and eating. Working with specialist therapists and the wider MDT to they will triage, assess and create individualised behaviour support plans and deliver early intervention parenting groups.

Paediatric Neurodisability Matron

The post holder will contribute to the provision of a learning disability service within the Tulip Paediatric Neurodisability Nursing team and share responsibility for assuring the quality of clinical practice within the team. Working alongside specialist Children’s therapy services at Croydon University Hospital the post will support a multidisciplinary tier 2 & 3 learning disability service for children and young people with moderate to severe disabilities who may also have additional needs such as mental health, behaviour and developmental disorders including ADHD and ASD.

You will manage your own caseload and provide comprehensive assessments and evidence based interventions. Post holders will be expected to contribute to the formulation of a child or young person’s care plan from a learning disability and neuropsychological perspective.

You will share responsibility for ensuring that staff within the team achieve positive clinical outcomes for children, young people and their families and that these are evidenced by performance indicators such as activity data, targets and quality measures.
