Discharge coordinator - Band 4 An exciting development opportunity has arisen to join our multi-disciplinary team as a band 4 discharge coordinator.
To independently actively monitor the multi-disciplinary management and discharge plan for all patients and act to expedite the process, avoid delays and thereby ensure efficient timely discharge. Taking an overarching role in supporting patient flow through the system. A critical success factor in this role is reducing the length of stay for all patients. To enable a patient to achieve their optimum level of independence and rehabilitation potential. DIMENSIONS Discharge support for 24 beds. Working closely with Physiotherapy, Occupational Therapy, Adult Social Care, nursing staff, Mental Health Liaison Nurses, medical staff and patient flow team. Telecare, District Nursing Teams, Reablement, Integrated Community Teams, GIS, relatives, care agencies. CORE KEY

The post is full time 37.5 hours.
