**About us**

**Your role**

Lago Homecare is seeking to bring an experienced Senior Carer into our team.

You will play a pivotal role in supporting our Registered Manager to manage our team of Carers, enabling them to deliver exceptional care to our service users.

You will build strong and trusting relationships with them, their families and loved ones, and the wider local community.

You will enable all of our service users to remain as independent as possible whilst maintaining their dignity and freedom of choice.

You will be based in our service user’s homes within the community, with some office time to support our Registered Manager with care administration.

**Your responsibilities**
- Performance and growth_
- Take ownership of individual service user care and support plans. This will include assessing and updating plans as well as delivery of personal care where appropriate. It will also include emotional and practical support, and recognising when there are changes in a service user’s needs that require discussion with the Registered Manager.
- Management of service user medication. This will include creating and maintaining eMAR charts. It will also include ordering, storing, and administering medication in a safe and effective manner, supporting weekly medication audits, staff observations and analysis of the quality of delivery by our Carers.
- Supporting the Registered Manager with the completion of a range of other audits, including quality, infection control, and health and safety.
- Regularly check our service user’s well-being as directed, and according to the needs detailed in their care and support, and risk, plans.
- Attend appointments with the service user, as necessary.
- As part of personal care delivery, you may need to carry out domestic tasks such as cleaning, laundry, and shopping.
- Assist with the preparation and provision of hot meals to our service users.
- Develop an awareness of other local community services and build effective and professional working relationships with them, including other health and social services.
- Sign-post our service users and their loved ones to local community services, where relevant.
- Deliver high quality, compassionate, person-centred care to our service users living with dementia, including supporting their health and wellbeing. Suitable, high-quality training will be provided.
- Contribute to effective team working and collaboration, so that you are part of a positive workplace culture.
- Work with the Registered Manager to carry out regular, constructive supervision and observation of your team of Carers.
- Use digital systems to ensure smooth delivery of service. This will include our care management software.
- Attend meetings and other activities as directed by your line manager. They may be outside of your regular shift pattern.
- Contribute to regular staff meetings. This may include presentation of specific topic areas as part of ‘Champion’ roles.
- Support your colleagues by ensuring visit reporting and handovers are clear, including review of those prepared by your team of Carers.
- Undertake any other tasks relevant to your role, as directed by the Registered Manager.
- Remain proactive and provide feedback to the management team about your own training and personal development needs.
- Regulatory_
- Appropriately respond to emergencies. Contact emergency services, deal with the breakdown of services e.g. power cuts, respond to death, and keep detailed and accurate records in all cases.
- Work with those from statutory and voluntary organisations to provide joined up care.
- Refer service users to external professional agencies where necessary.
- Advocating on behalf of service users with other agencies.
- Work within relevant health and safety standards. Raise any risks or issues in a timely manner with your peers or Registered Manager, using out of hours on call where necessary.
- Ensure that our administrative policies and procedures are followed and that records from our service users remain up to date.
- Deliver all services in accordance with the Care Act, Care Standards Act, and other relevant regulatory frameworks.
- Keep your professional knowledge and skills up to date, including on relevant legislation and mandatory internal training.
- Take part in our out-of-hours emergency on-call system.
