The Psychiatric Intensive Care Unit at Hergest Unit, Bangor consists of six beds and admits patients according to the national guidelines from across North Wales. The ward is fully staffed and there is input from professionals other than nurses, such as Occupational Therapists and Psychologists.

The Home Treatment Team (HTT) is also based at Hergest Unit and is a well-established team, covering Anglesey, North Gwynedd and parts of West Conwy. The total population is around 150 000. On average the team holds a caseload of around 15 patients. They work closely with the local Community Mental Health Teams in accordance with the HTT Operational Policy, which has been accepted by the colleagues and the Acute Care Forum.

A full-time middle grade doctor is allocated to support the post holder in covering the PICU and HTT.

To manage, appraise and give professional supervision to junior medical staff as agreed between consultant colleagues and the clinical director and in accordance with the Health Board’s personnel policies and procedures. This may include assessing competences under the Modernising Medical Careers framework.

To ensure that junior medical staff, working with the post holder operate within the parameters of the New Deal and are Working Time Directive compliant.

To record clinical activity accurately and comprehensively and submit this promptly to the necessary department.

To participate in annual appraisal for consultants.

To attend and participate in the academic programme of the Health Board, including lectures and seminars as part of the internal CPD programme.

To maintain professional registration with the General Medical Council, Mental Health Act 1983 Section 12(2) approval, to work within the legal framework of the Mental Health (Wales) Measure 2010 and to abide by professional codes of conduct.

To participate annually in a job plan review with the Clinical Director, which will include consultation in order to ensure that the post is developed to take into account changes in service configuration and delivery associated with modernisation.

To work with local managers and professional colleagues in ensuring the efficient running of services, and share with consultant colleagues in the medical contribution to management.

Acute Care is delivered from the three DGH based inpatient facilities at the Hergest Unit Ysbyty Gwynedd, Bangor, the Ablett Unit, Ysbyty Glan Clwyd, and Heddfan Unit, Wrexham Maelor Hospital. There are two PICU units based in Bangor and Wrexham. Acute care is led by acute care consultants within a collaborative model of care which maintains community consultant involvement and offers continuity.

Home Treatment Teams operate as part of Acute Care and are led by the Acute Care consultants. They provide the gateway for inpatient beds and in addition provide input to individuals who are suitable for early discharge from inpatient care.

The post holder will take clinical responsibility, including acting as Responsible Clinician for the patients admitted to the PICU in Bangor. This is a regional service and may accept admissions from all over North Wales.

The post holder will be a reference point for the multi-disciplinary teams taking an active part in ensuring that assessment, treatment and review procedures are consistent and of high quality.

Clinical duties include assessment of newly admitted patients including risk assessment and drawing up of management plans. The post holder will hold regular psychiatric reviews, attend team meetings, provide clinical supervision of junior doctors, clinical staff and medical students and engage in clinical audit.

The post holder will work collaboratively with the other Acute Care Services (open wards) to facilitate rapid transfer in accordance to the established care model. There is time within the weekly timetable for such liaison work to occur. Responsible clinician’s role will remain with the post holder during a patient’s stay at PICU.

Therapeutic input is provided by the inpatient Psychology Team, the Occupational Therapy Team and the inpatient based Physiotherapist.
