This is an exciting opportunity for an experienced clinician or social worker to work towards the delivery of the South East London LeDeR (Learning from lives and deaths of people with Learning Disabilities and/or Autism) programme. This is an existing post, developed in line with the new national requirements for LeDeR delivery.
The aim of the post is to play a key role in progressing local LeDeR review activity - complete reviews. The postholder will ensure that high quality reviews are conducted, working collaboratively with family members and those involved with the care of the person who died, with frequent interaction with primary care and social care colleagues.
Interviews will take place virtually via Microsoft Teams.

The postholder will ensure that learning from reviews is maximised and that findings are shared with local governance groups to inform service improvement activity across the ICS.
The post holder will be required to demonstrate excellent team working and interpersonal skills in order to develop and sustain robust partnerships with key stakeholders across South East London. The nature of the role requires an individual with a high level of emotional intelligence and an ability to build rapport and work sensitively and patiently with people who have been recently bereaved.
This is a hybrid post based in SEL ICB Headquarters (160 Tooley St, London, SE1 2QH) as part of the at-scale roles working across South East London. Due to the nature of this role, regular travel between sites and providers might be required.
The postholder will be expected to work confidently and collaboratively within high levels of uncertainty and ambiguity.

The South East London Integrated Care System (ICS) brings together the health and care partners that serve our vibrant and highly diverse populations resident in the London boroughs of Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark - our Places.
Our partnership brings together six local authorities, over 200 general practices (operating within 35 Primary Care Networks), Guy’s and St Thomas’ Hospital NHS FT, King’s College Hospital NHS FT, Lewisham and Greenwich NHS Trust, South London and the Maudsley Mental Health FT and Oxleas FT. Importantly, the ICS seeks to be connected to the communities we serve (circa 1.92m residents) and work with the widest possible range of community, voluntary and third sector groups and organisations in each borough. The reach of our NHS provider portfolios extends beyond the borders of the ICS, across London, the south of England and nationally for some services.

- Complete initial LeDeR reviews according to the standardised review process to the highest standards focusing on service improvement and adhering the to eight key principles of the LeDeR programme
- Liaise with bereaved families/carers following the death of a person with a learning disability to ensure the family can contribute effectively to the review.
- Identify, collect and collate health and social care records from social care, hospitals, GPs and other sources, with the assistance from the LeDeR Programme Support Officer.
- Transfer relevant information from the health and social care records, from the family and other sources into the LeDeR Review template.
- Present review findings at governance panels and support the panel to define Specific, Measurable, Attainable/Achievable, Relevant/Realistic, Time bound (SMART) actions offering professional challenge to ensure that the actions that are developed will achieve the needs of the findings of the review.
- Link with, escalate and refer concerns to other processes where necessary and advise families where another process (for example a Trust complaints process) would be more appropriate.
- Liaise with statutory processes such as safeguarding, medical examiners and the coroner’s office to ensure effective sharing of information.
- Arrange case discussion meetings as necessary.
- Attend and contribute to complex reviews meetings.
- Agree with senior reviewer and LAC any actions coming out of reviews.
- Input recommendations and lessons learned to the SEL LeDeR spreadsheet and theme tracker.
- Provide regular updates at the local Boroughs LeDeR Steering Groups on ongoing reviews, learnings and themes from completed reviews.
- Develop enhance working relationships across partner organisations.
- Maintain the programme content management system to ensure information is properly managed and best practice is shared across the team, SEL Learning Disabilities and Autism Programme and wider NHS and Local Authority system.
