To provide effective advice and support to the Physical Health and Rehabilitation Service and its patient pathways. The role will work alongside and deputise for exiting management structures to ensure that high standards of clinical business performance/service delivery
continue to deliver the highest quality of care to patients and their families across Cumbria.

On behalf of the ICC and Community Care Group, and in conjunction with the Specialist Services Service Manager, the post holder will take delegated responsibility for monitoring and ensuring that agreed commissioned services, SLAs and associated targets are understood, met, and, where necessary, support the delivery and implementation of plans to manage
clinical need.

To support, where necessary, the development of business cases, capital bids, project overview documents (PODs) and quality impact assessments (QIAs) to support service development initiatives and assist the management team in the business planning/continuous improvement cycle.

To support the maintenance and continuous improvement of patient care and experience. To work toward agreed service development plans, closely with clinical staff to and wider stakeholders, to ensure that departments runs efficiently and in line with NCIC/Care Group Objectives.

The role of the department is to provide community health based integrated care working alongside our partners in Primary Care and Social care.

The post holder will work across the Physical Health Psychology Service which includes nearly 100 posts, across 5 integrated services.
Within the team there are a range of Psychologists, Psychological Practitioners, CBT Therapists, Physiotherapists, Occupational Therapist, Living Well Coaches, Wellbeing Activators and Administrative Staff.
The department provides psychological care to patients with physical health problems, and associated psychological difficulties; this includes:
Long Term Conditions, including COPD, Diabetes and Cardiac problems, Cancer, Neuropsychology and Palliative care.

Persistent Physical Symptoms (persistent pain, Chronic Fatigue, Medically Unexplained Symptoms) including supporting patients with frequent attendance across the Health and Care System (High Intensity Users HIU).
The Service also hosts 1 the Wellbeing Service (supporting people within primary care setting with multiple Long Term Conditions or a change in life circumstances, and associated psychological difficulties), and 2 Paediatric Diabetes posts, striving to develop a paediatric Physical Health Psychology Service.

**The Service**:
Is integrated in Primary care working into MDTs.
Liaises with other core services for example First Step, Drug and Alcohol, Community.
Mental Health Team and CAMHS.
Works collaboratively with other teams and services.
