The Islington Fairer Together Borough Partnership provides a vehicle for uniting partners around a shared ambition for driving integration and tackling inequality in the Borough. We are dedicated to developing integrated localised approaches to service delivery underpinned by a philosophy that emphasises building relationships, building strengths, and building communities.

We are developing an ambitious strategy for helping people in Islington to Start, Live and Age Well. This is a proactive approach to fostering wellbeing through early intervention, prevention, a link to the wider determinants of health, strengths-based approaches, and the integration of health and care services.

This is a fantastic opportunity for a committed, energetic and creative individual to lead on our developments under Start Well, Live Well and Age Well agenda. The focus of this role will be on:
Age Well
- Domiciliary Care
- Intermediate Care
- Dementia
- Accommodation based pathways & Last Years of Life
- Carers
- Early Intervention and prevention

The role may also work on areas in Live Well portfolio as we encourage cross team working and collaboration.

Commissioning
1. To provide commissioning support on behalf of Islington Council in the development of plans and services for specified client groups.

2. To contribute to service reviews; identify strengths, weaknesses and gaps and report on these, developing proposals that ensure that services optimise health and wellbeing outcomes for service users and provide value for money.

3. To support a planned programme of service reviews, the validation and implementation of improvement plans as required.

4. To contribute to contract negotiations for new or remodelled services; contribute to producing annual contracting objectives and undertaking value for money assessments.

5. To adopt a project management approach to the completion of their commissioning work and to work to deadlines as required by the services.
Joint Working
6. To maintain effective working relationships with staff in other key agencies, in provider organisations and with the department’s front-line staff. Carry out all work in such a manner as to ensure that good public relations are maintained at all times.

7. To contribute towards the development and maintenance of relationships with all key stakeholders including users, carers, providers, commissioners, finance and information.

8. To provide advice and guidance to providers, service users and their representatives on contracting and funding arrangements.

9. To provide specialist advice to colleagues in adult services and with Voluntary and Community Sector.

10. To promote and specify continuous improvements in quality and outcomes through provider innovation and the development of personalisation and self-directed support.

11. To undertake consultation, engagement and coproduction with specific service user and carer groups to inform future service development and improve outcomes for service users.

12. To contribute to the quality assurance of services commissioned through liaison with services and contracts officers.

Performance Management and Legislation
14. To contribute to provider management and market development.

15. To contribute to the development of sets of performance indicators for services.

16. To contribute to the collection and collation of performance, activity and quality data on commissioned services to facilitate the production of accurate, informative and timely performance reports that meet departmental, corporate, partnership, and government requirements.

17. To comply fully with the Council’s systems including finance, procurement and legal governance, risk management, performance monitoring, information governance and staff performance management.

18. To keep abreast of new legislation, Government policy, best practice and of external factors relevant to the development of health and social care services.

Market Development
19. To support commissioning and market development initiatives on behalf of the Council.

Management Reporting
20. To produce management information, reports and statistics as required.

Other
21. To participate in meetings including inter-agency, multi-disciplinary meetings as required.

22. To attend meetings on behalf of the Commissioning Manager when required.

23. To undertake other duties commensurate to the grade of the post.

Additional
24. To use and assist others in the use of information technology systems to carry out duties in the most efficient and effective manner.

25. To achieve agreed service outcomes and outputs, and personal appraisal targets, as agreed by the line manager.

26. To undertake training and constructively take part in meetings, supervision, seminars and other events designed to improve communication and assist with the effective development of the post and post holder.
