Health Referral Facilitator

GLL is looking for a Flexible GP Healthwise Referral Instructor to work in our Cornwall Partnership at Truro Leisure Centre. The GP Referral Instructor is an important role in our Cornwall Partnership to help us deliver our health and wellbeing objectives in our Lewisham Partnership. The GP Referral Instructor will be GLL’s key contact to link in with GPs and Health Professionals in the borough as well as working alongside the local authority. Working with a small team the GP Referral Instructor will manage and oversee successful delivery of the Healthwise programme.
**What you’ll do**:
- Manage the Healthwise Programme.
- Be the key contact for GPs/Health Professionals and the local authority
- Complete practical as well as administrative elements of Healthwise
- Represent Healthwise in internal and external meetings.
- Produce monthly, quarterly and annual reports.

**What you need**:
- Level 3 GP Referral qualification
- NVQ Level 3 Fitness Instructor
- REPS registered fitness professionals with patience, empathy, sensitivity
- Experience of delivering and managing lifestyle interventions.
