Multiskilled Primary Care Worker

**Key Duties and Responsibilities**: Personalised Care Work with a cohort of patients as identified by the Practice/PCN to deliver personalised care. Use data and a population health management approach to focus on people most in need of support. Support the process of bringing together all of a persons identified care and support needs and explore options to meet these within a single personalised care and support plan (PCSP). Assist people to access self-management education courses, peer support or interventions that support them to take more control of their health and wellbeing.
Provide co-ordination and navigation for people and their carers across health and care services to ensure patients receive appropriate support from the right person at the right time. Identify patients who may benefit from shared decision-making and support Practice/PCN staff to facilitate shared decision-making conversations. Support the Practice/PCN in developing communication channels between GPs, patients, families and carers. Maintain accurate records of referrals and interventions to enable monitoring and evaluation.
Answer telephone and deal with or redirect enquiries/messages to relevant colleagues. Record requests for home visits and contact duty doctor with late calls. Receive requests for repeat treatment and repeat prescriptions as directed by Practice policy. Deal with mail as per Practice procedure.
Extract medical records or print computerised records when required for surgery consultation, home visits, etc. Arrange urgent transport, nursing or Social Services liaison. File medical records, hospital reports and letters. Maintain stationery and medical supplies available in reception area and surgeries.
Promote own personal and others health, safety, and security, taking the appropriate action to report risk in accordance with policy. Ensure that equipment and resources are managed effectively to reduce waste and promote cost efficiency. Ensure the safe keeping and maintenance of equipment in accordance with policy, reporting defect or loss. Maintain personal development to meet the changing demands of the job, participate in an annual appraisal and appropriate training activities.
Ensure compliance with all Kernow Health CIC statutory and mandatory training requirements, policies and protocols. Take appropriate responsibility to ensure that your objectives are aligned with the core team and organisational objectives. To demonstrate agility through adapting to the needs of the organisation by working flexibly in response to changing organisational requirements and priorities. You will be expected to carry out any other duties that may reasonably be required in line with your main duties, as directed by your line manager.
