As a result of funding from the AFCT, we have an exciting and rewarding opportunity available for an experienced health and social care co-ordinator for an on-site Veterans Wellbeing Navigator (Complex Care) to join our team at Broughton House Veteran Care Village in Salford Greater Manchester. Engaging primarily with working age veterans with multiple complex needs you will provide information and support to ensure these veterans access the support they need from specialist providers across the Greater Manchester. This is a full-time, 3-year fixed term contract.

We are seeking an individual with a compassionate and flexible approach to welfare delivery, who is a highly organised, analytical and an articulate communicator to provide advice and information on a 121 basis. Your primary role will be to help veterans and their families to navigate the often, complex pathways of accessing statutory and volunteer health and welfare services by providing information and support and “a holding hand” when necessary to identify and engage with appropriate statutory and or voluntary services within the community to meet these veterans multi-disciplinary/agency social care, health & welfare needs in a joined-up manner.

**The Navigator will**:

- Meet each person face-to-face, undertaking a holistic assessment to understand their military service and the range of challenges they face.
- Help each person develop a goal-orientated personal action plan, identifying areas of life they need support and the outcomes they wish to achieve.
- Identify the most appropriate organisations to help the veteran with each aspect of their plan (e.g., benefits, debt, mental health, addictions, unemployment, isolation, and/or housing).
- Over a period typically lasting 6-months to a year, support and advocate for the veteran and their family, working alongside identified providers ensuring the veteran receives the holistic, person-centred support they need.

The Navigator’s support will vary according to individual needs, but is likely to include help/support to:

- understand welfare benefit entitlements and complete claim forms, escalating to specialist providers (e.g., Citizens Advice Manchester) where needed.
- access the most appropriate mental health service (e.g., Operation Courage, Military Veterans Service, or specialist counselling).
- address drug/alcohol/gambling addictions (e.g., Tom Harrison House).
- find employment/training (e.g. Walking with the Wounded; Forces Employment Charity)
- address housing issues (e.g., Salford Housing)
- reduce social isolation/improve wellbeing (e.g., Royal British Legion, Help for Heroes, SSAFA)

The Navigator will build each person’s confidence and self-advocacy capacity, accompanying them to initial meetings where needed, and continuing to advocate for and support them until they’ve achieved their personal plan objectives. As a result of support through the project, people will receive better co-ordinated, multi-agency support, leading to better outcomes.

- With strong organisational and time management skills have an ability to work with minimum supervision
- Strong administrative and database use with good Microsoft office and keyboard skills
