**Medical Records Clerk**

**Role: Medical Records Clerk**

**Pay: NHS Band 2 - £9.50ph**

**Hours: Office Hours Monday - Friday. Full & Part-time Available. Hours agreed directly. There will be a requirement to work at least one evening per week where the hours will be 17.00-22.00**

**Location: Norwich**

**General Duties**:

- To retrieve and supply of case notes requested urgently by telephone for A&E, Emergency Admissions and outpatient services
- To retrieve and supply all case notes for planned outpatient, in-patient and other specified services
- To receive, maintain and file all case notes and assorted documents
- To accurately record all case note movements into and from the Health Records Library
- To maintain manual records of all work undertaken
- To deal with telephone requests and enquiries and to re-direct as appropriate

**Specific Additional Responsibilities**:

- To deal with notifications of death and record on PAS in accordance with departmental procedure
- To provide information from patient case notes to other departments, hospitals and outside agencies according to agreed policies and with appropriate authorisation
- To retrieve and supply case notes required for audit or research purposes, according to agreed policies and following authorisation by the Line Manager
- To maintain deceased patients’ and other archived medical records in accordance with the Trust’s Records Management Information Lifecycle Policy
- To move records between the live, deceased and overflow stores as required
- To record and deal with requests from other departments for supply of case note folders and associated items
