Cambridgeshire County Council are looking for a qualified Social Worker to join thier Adults and Autism Team. £37ph. This is a HYBRID role. You will be working 2 days a week in the office but flexible based on demand

Your role will be working with adults up to the age of 75 with Autism (and no associated learning disability) or other needs which do not fall to other statutory social care teams to complete Care Act assessments,

**Responsibilities include**:
To determine eligibility for support and consider creative and flexible ways to meet this through the development of care and support plans. To complete carers conversations and, where required, carers assessments and reviews.

To complete reviews for those in receipt of services, or with assessed needs where these are met via alternate sources of support. To contribute to the teams duty system, undertaking calls to those on our waiting list for allocation and managing contact from a wide range of individuals and professionals with regard to those we support.

To undertake making safeguarding personal enquiries if required and to raise any safeguarding concerns appropriately.

To manage own case load, proactively seeking advice and guidance as required and coming prepared to monthly supervision. To adhere to required legislation and policies and procedures
