**About the Role**:
Join Oldham Council as a Family Support Worker and play a vital role in providing support to families and children with disabilities. This position requires a Short Breaks Worker who will be responsible for conducting short breaks assessments, reviews, and carers assessments to assess children with disabilities for short breaks respite/PA support.

**Key Responsibilities**:

- Conduct short breaks assessments for children with disabilities to determine their respite care needs.
- Perform short breaks reviews to evaluate the effectiveness of existing support plans and make necessary adjustments.
- Conduct carers assessments to assess the needs of family members or carers providing support to children with disabilities.
- Collaborate with families and carers to develop comprehensive support plans tailored to the individual needs of children with disabilities.
- Provide guidance and advice to families on accessing short breaks respite and personal assistant (PA) support services.
- Work closely with other professionals and agencies involved in the care and support of children with disabilities.
- Maintain accurate records of assessments, reviews, and support plans in accordance with organizational policies and procedures.
- Ensure that the welfare and best interests of children with disabilities are prioritized in all decision-making processes.

**Requirements**:

- Previous experience in a similar role, ideally within a social care or disability support setting.
- Knowledge of short breaks assessment processes and procedures.
- Strong communication and interpersonal skills, with the ability to engage effectively with families and carers.
- Ability to work collaboratively as part of a multidisciplinary team.
- Excellent organizational skills and attention to detail.
- Understanding of safeguarding principles and procedures.
- Flexibility to adapt to changing priorities and workload demands.

**Salary**: Up to £14.79 per hour
