Clinical Through triage and advanced clinical assessment identify the most appropriate support and treatment for the patient To refer patients to alternative services as determined by their clinical need For those patients accepted into the Hospital at Home Service to develop a personalised care plan including predicted date of discharge. To improve the quality of care received and the experience of patients and their carers through implementation of initiatives which reflect the promotion of independence. To ensure care is delivered by the most appropriate member of staff and resources are used to best effect To supervise and support team members, formal and informal carers in the delivery of clinical care To deliver a range of advanced clinical skills including physical examination, venepuncture, cannulation and IV therapy. To participate in daily monitoring of care plans to ensure discharge dates are met wherever possible To keep and maintain accurate documentation and clinical records in line with PML Policy, NMC and HCPC Codes of Conduct and Data Protection Act.

To manage the competing clinical priorities of the caseload, taking necessary action including seeking appropriate clinical or managerial support. To develop a culture of anticipatory and self-care where appropriate To develop and sustain close and effective working relationships with partner services, statutory, and voluntary organisations in particular the Gerontology and Out of Hours Medical services, SPA, Reablement Care, Community Rehabilitation, Community Nursing Service and Specialist Nursing and Therapy services. To develop and sustain close communication links between key partner services to ensure that information is shared and acted upon, resulting in optimal levels of patient care To develop the use of technology to support patients remotely as appropriate. Management responsibilities To support Clinical Lead in ensuring the service is appropriately staffed with in the core hours of 8am 10pm daily.

Attend county wide capacity meetings to identify potential workload and allocate as necessary. To work with the Clinical Lead to ensure team members are appropriately skilled to deliver the service. To work collaboratively with the Clinical Lead to ensure that teams training and appraisals are completed. To deputise for the Clinical Lead and encourage all staff to actively contribute to the ongoing service development.

To work in partnership with the Clinical Lead and Governance team to develop new clinical guidelines and protocols as required by the service To ensure all service specific data is submitted in a timely manner. To assist with reporting timelines and deputise for Clinical Lead at governance and management meetings. To ensure that key performance indicators are considered and met. To ensure information sharing processes between partner services and agencies is within the legislative framework.

To support the patient and partner service experience surveys process. To assist in the investigation of any incidents or complaints relevant to the service Health & Safety To take responsibility for the health & safety of themselves and other persons who may be affected by their omissions or actions at work. To promote the Health and Safety policy and ensure matters are managed in accordance with it. Report accidents, incidents and near misses, implementing corrective action where necessary.

Infection Control and hand hygiene To comply with PML policies for infection control and hand hygiene such as hand hygiene, to reduce the spread of healthcare-associated infections (HCAIs). Employees with clinical responsibilities must incorporate into their clinical activities up-to-date evidence that supports safe infection control practices and procedures, such as the use of aseptic techniques and the safe disposal of sharps. Personal/Professional development Participate in the companys mandatory training programme and an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development. Take responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.

To participate in relevant local clinical audit and research to inform service delivery and development. To be accountable for prioritisation of own workload. To attend relevant internal and external meetings as agreed with the Clinical Lead. To actively participate in a clinical supervision programme To demonstrate leadership skills that can influence service improvements and support service delivery.

To actively participate in an annual performance review (appraisal) and the development and implementation of a personal development plan. To take responsibility for own professional development ensuring professional standards are maintained and statutory and mandatory training is in date. To attend any training as requested. Finance
