Frailty Senior Support Worker, Band 4

Demonstrate knowledge of and undertake assessments of patients with non-complex needs e.g., Activities of daily living, mobility and transfers, using therapeutic techniques and principles of Occupational Therapy and Physiotherapy. Using these skills and knowledge develop a plan for discharge using clinical skills within level of band 4 competency. - Encourage patients to engage in assessments and encourage maintaining independence where appropriate in order to reduce deconditioning in hospital environment. -Demonstrate knowledge of and ability to effectively use, assess for and provide appropriate equipment.
- Demonstrate an excellent knowledge of manual handling techniques and risk assessment. - Have knowledge and experience of general medical conditions and or clinical interventions pertinent to a caseload of frail older adults. - Effectively communicate with patients, carers and relatives verbally (face to face or telephone), in written form and non-verbally. -Ability to take into account and recognise situations that are distressing or emotional and respond accordingly.
- Communicate effectively and as an advocate where the patients ability to communicate is compromised. Using effective interpersonal skills to communicate with patients and relatives to provide patient centred care. - Maintain effective communication links with all levels of nursing and other health care professionals. - Awareness of barriers to communication (e.g., perceptual, cognitive, physical) and employ strategies to overcome these.
- Ensure confidentiality and dignity is respected and maintained at all times. - Maintain accurate, comprehensive and timely clinical records of all care, advice and support delivered with clear reasoning for interventions in accordance with trust and national guidance. - Plan and prioritise own workload in accordance with therapy standards and in conjunction with the responsible therapist. - Monitor, assess and analyse progress and report back to qualified staff at regular intervals, participating in daily huddles.
- Demonstrate ability to work alone in domiciliary settings when required, assessing and managing risk.
