Accredilink are looking for personable, highly motivated, caring individuals to join our expanding dedicated care support teams in the Denbigh area. As a member of our team you should be passionate and highly focussed providing person centered care with an emphasis on promoting as much independence as possible within the persons own home environment.

The Community Healthcare Support Practitioner Role is key to providing the very best care and support services available.

This Role includes but is not limited to:

- Providing Personal Care
- Assisting with day to day living tasks.
- Mental Wellbeing support.
- Support with medication administration.
- Following and implementing written Care and Nursing plans.
- Maintaining accurate Support Records
- Maintain Confidentiality.
- Liaising with Family, Friends and other Healthcare support networks.
- Taking basic observations (blood pressure etc) in order to calculate a NEWS 2 score to dictate a care pathway in a medical emergency.
- Monitoring any medical, nutritional, social changes in circumstances and reporting accordingly to effect a positive outcome
- Co-working with District Nurses, Social Workers, Occupational Therapist and other Healthcare professionals
- Use of Moving with Dignity Aids
- Being a listening ear and provide a friendly chat

Previous experience desirable but not required as full training will be provided.
