The Trainee Clinical Endoscopist will be required to complete a training programme of approximately 18-24 months duration. This will include attendance at training courses, completion of practical assessments, JAG training and accreditation by JAG as an independent Endoscopist for both upper GI and lower GI endoscopy procedures.

The training includes attendance and completion of the following courses on specified dates throughout the training period:

- JAG basic skills
- JAG hands on Therapeutic course
- Basic Cancer Training
- Advanced Communication Course
- National Nurse Endoscopist course
- Non medical prescribing course
- NHS Academy leadership Programme
- Clinical Leadership and Management Development Programme University of Bedfordshire. The post holder will be mentored by a nominated consultant gastroenterologist on a weekly basis and will attend regular supervision sessions

The post holder will be allocated specific weekly training lists to undertake supervised scopes as part of the training programme within the agreed time period, a proportion of which should be unassisted. Evidence will be required of the completion rate/outcomes of completed endoscopies, including patient vasovagal episodes, bleeding problems, unplanned admissions and the use of reversal agent. The audit should be verified and signed off by the Endoscopy Clinical Operations Manager and by the lead consultant gastroenterologist for endoscopy.

The post holder will work with the MDT and contribute to the development, management and delivery of a high quality general and specialist mixed GI endoscopy service.

The post holder should be observed by their mentor undertaking endoscopies and there is a requirement to submit formative assessments during the training period.

The post holder will work in partnership with the MDT to ensure that local practice is of the highest standard for current and future patients whilst achieving all national and local targets. At least 70% of the duty time will be spent in providing direct clinical care and advice for GI patients at the diagnostic stage of their care. This involves the utilisation of highly developed specialist knowledge, critical thinking and expert decision-making skills in order to assess, plan, implement and evaluate programmes of care for this cohort of patients.

On completion of the training post, the trainee will be upgraded to a band 8a position in accordance with defined protocols. They will undertake diagnostic and therapeutic upper and lower GI endoscopy procedures using advanced and specialist clinical skills as an independent autonomous practitioner - providing advice and identifying the need for further investigations as required. They will be able to administer conscious Midazolam sedation and analgesia and be responsible for the development and amendment of departmental policies related to conscious sedation in accordance with British Gastroenterology.

1. 1. Take responsibility for the clinical management of the patient during the procedure including pre procedure assessment/ the procedure and recovery.
2. Take informed consent prior to the procedure in accordance with Trust policies and ensure adherence to annual update of consent training
3. Using highly specialised knowledge ensure that patients and carers receive appropriate health education pre and post procedure
4. To have undertaken/ or be undertaking a formal approved course in endoscopy training recognised by JAG and maintain competency and knowledge required for this role
5. Prescribe medication as defined in the PGD framework
6. Communicate bad or potentially distressing news to patients in a sensitive and timely manner and provide support and point of contact to patients during the treatment pathway. Promote patient empowerment to enable informed choices through evidence based education and advice
