Hospital Discharge Support Assistant - Chelmsford - Monday to Friday
Hays are proud to be working on behalf of a large and reputable charity who are looking for a Hospital Discharge Support Assistant to join their team on a fixed term contract until the end of March 2023. This role is working Monday to Friday 9am - 5pm with1 hour lunch break.
This position involves picking vulnerable individuals up from hospital once they have been discharged (this could be for any reason) and transporting them home. You will then be responsible for undertaking a risk assessment within their homes to make sure theyare safe. Making any adjustments if necessary,
Within this role you will also be responsible for transporting medication and equipment from the local hospital to relevant individuals homes.
You will be responsible for providing emotional care and support to these individuals to make them feel safe and comfortable.
To be considered for this role, you MUST drive as this role is 80% driving. You MUST also be living in the UK and have right to work here as we are unable to offer sponsorships for this position.
This is a temporary contract until March/April 2023 so you must be able to commit to this assignment along with full time hours of 9am - 5pm.
