Job summary

Exciting opportunity to join the newly developed and evolving MSE Discharge to Assess (D2A) team at Broomfield Hospital.

The aim of the service is to create a smooth pathway from the acute hospital into the community by assessing the patient's therapy and care needs in their own home. This means that patients no longer need to wait in hospital for these specific assessments which will reduce delays discharges, improving patient flow and allowing patients to go home earlier with more therapy wrap around support.

The team is based at Broomfield Hospital and provides post-discharge early intervention into patients' homes.

The team fosters a supportive environment that seeks to develop therapists and provide them with the skills to work within the new D2A model in the home environment.

The Team is benefiting from focused support and leadership and established relationships with the Therapists within the acute hospitals locally. MSE has a focus on developing the clinical and non-clinical skills of its staff both personally and professionally.

****Due to the nature of the role the successful candidate would be required to be a car driver with access to a car of work purposes***

Main duties of the job

You will be responsible for undertaking assessment in the community to identify equipment and care needs at home and amend them as necessary. There is opportunity for working through early goals for patients under the Discharge to Assess (D2A) pathway, including kitchen tasks and other ADLs in the persons own home.

You will coordinate services by undertaking a person-centered assessment; developing, implementing and evaluating programmes of care, rehabilitation goals and assessing equipment needs.

You will work collaboratively with the Primary Health Care Teams, Social and Voluntary sectors to set up packages to enable patients to be cared for at home. Post holder will work autonomously within professional guidelines and the overall framework of the service's policies and procedures.

The team is a multi-skilled team and all staff will receive training to be multi-skilled to provide comprehensive assessments and treatment of patients. This may also include assessing and planning Health and social care packages in agreement with the MDT. All these skills are competency led, and appropriate training will be provided.
