We are looking to recruit highly motivated clinical support workers to work within one of our integrated community nursing teams, supporting the Continence service.

This role requires an enthusiasm for Continence and interest to support patients to manage their condition, it will involve direct care with clients (within their own homes, care homes and clinic environments), assisting with clinical procedures and undertaking delegated duties unsupervised. You will be part of a team providing high quality nursing care and supporting the case management for patients with long term conditions and associated Continence needs.

To work in collaboration with ‘Place based integrated community nursing teams’, the role supports the work of the Continence service for patients in the community and other settings.

The post holder will be supported and supervised by the senior clinician, but will work regularly without supervision in the delivery of clinical procedures and undertaking delegated duties.

This role supports the proactive monitoring of clients in order to promote and sustain their independence and to prevent unnecessary hospital admissions.

To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience in the community, ensuring the highest standard.

The post holder will highlight risk and changes in patient’s circumstances / condition by effective communication with registered practitioners.

Be expected to implement and deliver a care plan, undertaking assigned procedures as directed, working within the boundaries identified by the registered practitioner.

Demonstrate excellent communication skills with clients, relatives, carers and colleagues.

Have an understanding of the roles of the multidisciplinary team and support services available within the community.

Maintain excellent written and electronic patient records.

To undertake nursing procedures and treatments as required, working within outlined lower level clinical competencies and Trust protocols, as directed by the senior clinician.

To work in partnership with patients, relatives and carers and the wider multi-disciplinary team in developing assessment, planning, implementation and evaluation of care.

To support and enable clients to maintain their health needs.

To deliver programmes of health education and health promotion as required, such as smoking cessation, diet, exercise, pulmonary rehabilitation.

To undertake appropriate risk assessments and take the appropriate action which may avoid foreseeable harm occurring to patients in their own home.

To inform the senior clinician of all changes In relation to patient care.

Identify changes in patient’s conditions and recognise within their capabilities where significant changes in the care plan are required and report these back to the senior clinician, facilitating a step up approach to care delivery.

To work within and continue to develop knowledge and skills for lower level clinical competencies as identified for the role.
