Dementia Support Worker/activities Organiser

The post holder will work as part of the multi-disciplinary team in order to plan, organise and facilitate a comprehensive range of individualised, group and one to one activities for the inpatients in the acute care of the elderly wards. They will promote and encourage appropriate, individualised level of patient participation and facilitate patient activities that promote social interaction, enhance self esteem and help to maintain or enhance functional ability. They will support the multi disciplinary team in the assessment of patients with functional disorders and ensure their nutritional status is monitored.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
To provide a social environment and relevant activities appropriate to patient needs with minimum supervision
To work in collaboration with other staff
Maintain a safe, clean working area and ensure that all tools, materials and equipment are appropriately used and stored and are in good working order adhering to Infection Control guidelines
Participation in the organisation of the unit in conjunction with nursing and other colleagues to ensure the ward environment meets patient needs
To liaise with other Activities Coordinators and Occupational Therapy staff to enhance the scope of activities available to patients on the unit
To recognise and respond appropriately to any changes in patients presentation, behaviour or condition.
To report to qualified staff as necessary on misadventures or accidents involving patients or staff
In collaboration with the ward manager help implement the falls care bundle in order to reduce inpatient falls
Introduce a range of interventions to reduce falls and monitor the impact of these interventions
To participate in the units appraisal scheme To participate in in-service training as required
To work with the nursing team, patients and carers to ensure accurate completion of the ‘this is me’ information brochures.
