Purpose

You will work with people with a physical and/or mental health condition which impacts their ability to gain or sustain work. You will manage a caseload of between 25 to 30 individuals to assist them in securing sustainable paid employment in line with their preferences. You will deliver the Individual Placement and Support (IPS) approach (for which training will be given); providing person centred advice and guidance to individuals, whilst building positive relationships with local employers and stakeholders to enable individuals to move into suitable employment. Embedded within Primary Care, GP practices, Community Health Teams, Job Centres and the wider community, you will support participants on their recovery journey towards better health and wellbeing.

IPS delivery experience (preferably in a Primary care setting)

Proven experience of supporting people to obtain or keep work

Proven experience of meeting and exceeding outcomes and targets

**Location**:
You will be based in Sheffield and will be required to travel to different locations to meet your clients.
