**About**
New Chapters is a family run service that offers vulnerable adults the opportunity to live independently but in a supported environment. We are passionate about providing the personal touch for the families we work with and the individuals we support as we endeavour to be more than just a support provider but to be an integral part of the journey for the people we have the privilege to support.
The service offers the opportunity for individuals to live within supported accommodation, and to have the choice of staff support with maintaining welfare and choice, support with daily living skills, support with emotional needs, whilst maintaining a level of independence which meets the assessed needs of the individual. The individual will have the opportunity be supported with managing their own tenancy, finances, maintaining access to the community and building their future.
“New Chapters” wants to offer a managed service that offers a future to those who need it and be able to see individuals gain self-respect, confidence, skills, education, and prospects that are comfortable and at a level that they feel is possible for them to manage independently and in the community.
**Role Purpose**
The role will be to deliver 1:1 and core support to support young adults with learning disabilities, autism spectrum disorder and/or mental health needs.
This role is about support our young service users towards becoming more independent and self-sufficient with a key focus on wellbeing and good mental health.
We are looking for those who can offer our young service users a role model in leading a proactive and healthy life they can enjoy.
**Tasks, Duties, And Responsibilities**
1. Support with day-to-day needs including cooking, cleaning, and shopping
2. Encouragement to maintain good personal hygiene
3. To provide an emotionally secure, supportive, homely and caring environment, appropriate to the social, cultural and religious needs of all the people we support in the home.
4. Support with managing tenancy, benefits, and finances
5. Support to access the community & participation in community projects
6. Support with medication (where required)
7. Support to access GP and other health appointments
8. Support to live & maintain a healthy lifestyle
9. Support with accessing education and employment
10. Overnight support (sleep-in) when required
**Generic Duties & Responsibilities**
1. To represent and promote our values acting as an ambassador on behalf of New Chapters Support & Wellbeing
2. To actively promote and act, always, in accordance with New Chapters Support & Wellbeing policies, including, but not limited to: Health and Safety, Equal Opportunities, Prevent and Safeguarding, the Staff Code of Conduct.
3. To participate in New Chapters Support & Wellbeing Annual Appraisal Process, contributing to a culture of self-reflection on practice and continuous professional development
**Other Reasonable Duties**
This Job Description sets out the main duties of the post at the date when it was drawn up. Such duties may vary from time to time without changing the general character of the post or the level of responsibility entailed. Such variations are a common occurrence and cannot of themselves justify a reconsideration of the grading of the post. In cases, however, where there is a permanent or substantial change in the duties and responsibilities New Chapters Support & Wellbeing reserves the right to amend the job description in consultation with the employee to reflect changes in the duties of the post. This list of duties is not exhaustive and from time-to-time staff will be asked to undertake any other reasonable tasks in relation to their role.

All new employees to New Chapters Support & Wellbeing are required to complete and obtain an enhanced DBS disclosure. If the above clearances and references are not consistent with the information provided by you then your offer of employment may be withdrawn.
