Dementia Navigator will work in a fully integrated way as part of City and Hackney Dementia Service multidisciplinary team to help maximise identification, engagement and support for service users, carers and families with dementia illness. This will enable service users to live well in the community for as long as possible. They will also work with the Dementia Alliance and other local organisations/agencies to help make City & Hackney a more dementia friendly place to live

The Navigator will offer comprehensive information, support, and advice around the processes for screening / diagnosing dementia, the benefits of engaging with the service to facilitate timely diagnosis, what to expect during the screening and diagnostic processes and the support they can offer.

Voluntary, Community and social Enterprises (VCSE) organisations are important in maximising engagement/uptake, particularly linked to Mental Health services where they play a key role in reducing stigma. Navigators will attend joint meetings and events organised by voluntary care sector enterprises to provide information on dementia, promote the service and its benefits, and encourage / facilitate referrals

Maintain and effectively manage a caseload, providing an action plan to people affected by dementia that enable them to adapt to changes they may experience and provide appropriate support based on their changing needs.

To provide structured on-going support and follow up reviews tailored to the needs of person affected by dementia as required

To complete Risk Stratifications that will inform and identify ongoing needs, intervention and risk management.

To maintain timely and accurate patient written / electronic records under supervision and in line with Trust and service line policy and procedure

To work within a multidisciplinary team, following plans of care and reporting to a more senior member of staff to undertake a range of duties and interventions. This may involve assessing, planning and implementing interventions in negotiation with service users, carers and other services taking into consideration the needs of a diverse community.

To be aware of and assist in meeting the psychological, social, physical, recreational and spiritual needs of all service users. To recognise and value service users as individuals, recognising the importance of maintaining the service users dignity at all times. To ensure that the care provided reflects respects, equality and diversity that is supportive of service users and their carers.

Encouraging service users to accept an optimum level of responsibility for their individual programme of care and with their consent.
