An exciting opportunity has arisen for a committed, enthusiastic individual to join our forward-thinking Community Teams in a new post as a **Trusted Assessor **for our Integrated Discharge Team.

You would be joining our Community Division at an exciting time as we progress on our journey to outstanding, having achieved a CQC rating of good overall in 2019 including Outstanding for Care in 2021. Our transformational achievements include the extensive use of technology enabled care and the development of integrated multi-disciplinary and multi-agency teams, in conjunction with a wide range of partners across the Island and mainland. The team has a wide breath of knowledge including Occupational Therapy, Physiotherapy, Social Work and Nursing.

A recent organisational change has established a new collective leadership and management structure across our Community and Mental Health and Learning Disabilities Divisions. This change is transformational for both divisions, aiming to establish a more resilient service management structure to better support our new models of care in terms of multi-agency and multi-disciplinary working, as well as creating capacity for development of the partnership between both divisions and our wider partners across the local system.

The post holder will be passionate about hospital flow and the 'home first approach'.
