An excellent opportunity has arisen for a discharge coordinator to join our Stroke team at QMC campus. The post holder will be based on C5 and expected to coordinate inpatient flow across the three wards in stroke services. They will work in collaboration with the Medicine bed managers and the SMT (specialty management team - head of service, matron and specialty general manager) as well the wards staff and MDT team.

Main duties of the job

The focus of the role is to ensure timely discharges and transfers. Escalate any issues that occur during a patient’s journey and delays discharge.

A proactive approach is required to pre-empt delays where possible by using their knowledge and understanding of stroke services and hospital systems such as CRIS/Medway and Nerve Centre in order to monitor the patient flow and identify issues. The post holder will inform the Nurse in Charge and Consultant in Charge of any internal escalations that hinder patient flow through the departments.

The Stroke Discharge Coordinator role is seen by all clinical and nursing staff as an integral role to the patient journey and support to staff in Stroke.

The key responsibilities are to ensure timely discharges and transfers, attend MDT board rounds, liaise with bed managers, relatives, care homes and discharge lounge. Keep hospital systems such as Nervecntre up to date with key information.

**Salary**: The quoted salary will be on a pro rata basis for part time workers.
