Our discharge coordinators support the Nurses by completing non clinical tasks such as booking Transport, liaising with diagnostic specialist or talking to relatives. The discharge coordinator works closely with our integrated discharge team and social care colleagues.

Main duties of the job

In addition to the below summary, you’ll need to familiarise yourself with full Job Description and Person Specification documents attached to this advert.

The post holder will have awareness of and escalate delays along a patient’s journey, working as required to minimise these.

The post holder will work proactively, pre-empting delays where possible, and helping to plan for discharge when a patient is ‘maybe-home’.

The post holder will inform the Nurse in Charge and Consultant in Charge of any internal escalations that hinder patient flow through the departments.

Familiarized with D2A documentation and other referral forms for discharge.

Detailed job description and main responsibilities

KEY JOB RESPONSIBILITIES

To attend nursing handovers (7am)
To attend Southwell Huddle (9am)
To attend board rounds (11.30am, 4.30pm).
To liaise with overnight Juniors and Registrars for details of any patient who is likely quick-home. To have an overall understanding of what all patients are waiting for, and of any delays.

Speak with relevant Specialties and teams when there is a delay, i.e. radiology, STOC, DPM, alcohol liaison, Cardiology.
To highlight any actions to the medical, nursing and pharmacy team where required to prevent a delay, i.e. to review a CT report, check blood results, complete TTO.

Have an awareness of patients in ED and RAU awaiting a Southwell bed, their length of stay in the department, and any upcoming 12-hour bed breaches. To work with Nurse in Charge to plan to create capacity for these patients.
Have awareness of bed capacity, in regards to when capacity is expected for patients awaiting transfer, and when reserved beds become empty. Liaise with bay nurses to ensure patients are ready for transfer when beds become empty.
Book transport for patients transferring to other hospitals, or home as required. To link with the Site Matron team should there be any issues or delay.

To ensure patients have a timely discharge, informing the patient, liaising with relatives and care homes, and sending the patient to the Discharge Lounge where possible.
Prepare for the following day’s expected discharges, ensuring as many pre-noon discharges as possible.
Support clinical teams where required to ensure Nervecentre remains up to date with key information following doctor decision, such as specialty tag, bed request or home today.

Support the use of Nurse-Led Discharge, to prevent the need for a further senior review.
To ensure ‘Home Today’ is up to date on Nervecentre for every patient.
Ensure completion of Friends & Family tests and general inpatient surveys.
Answer the phone at the nurse coordinators desk, and deal with queries as appropriate.
To be welcoming and helpful to all staff, patients and visitors in person and on the phone.
Provide relevant information to patients, family/visitors/carers and colleagues.
Maintain patient confidentiality.
Maintain professional boundaries and working relationships with patients and colleagues. Understand own level of responsibility and accountability in relation to team structure.
Any other duties reasonably expected of a Band 3 post holder at NUH.
