The Sussex Virtual Health programme is embarking on its journey, working alongside our system colleagues to deliver the Sussex Virtual Health Model, integrating Primary, Community and Urgent Emergency care across Sussex. The programme will deliver the objectives within the Sussex Shared Delivery Plan - Improving Lives Together.

We have an exciting opportunity for two experienced Band 8a Senior Service Change Partners to programme manage the Virtual Health programme, with the roles being hosted under the leadership of Sussex Community NHS Foundation Trust. Both roles will be system-wide roles across the Sussex Health and Care Partnership.

**Two roles available**:

- Senior Service Change Partner to programme manage the Virtual Health workstreams - this role will be responsible for the robust programme management of primary care, community and UEC transformation and integration to deliver Virtual Health
- Senior Service Change Partner to programme manage the Digital Delivery workstreams - this role will be responsible for the transformation and programme management of Virtual Care, delivering digital capability for Virtual Health

Both roles will need to work closely together to ensure delivery of the overall programme to competing deadlines.

We are looking for two experienced programme managers who are competent to work and deliver at pace system-wide. You will have excellent communication and engagement skills, and bring energy, enthusiasm to change, and the ability to influence.

The Senior Service Change Partner (system roles) will support the Sussex Virtual Health programme to deliver the Sussex Virtual Health Model, integrating Primary, Community and Urgent Emergency Care across Sussex, and delivering the objectives within the Sussex Shared Delivery Plan - Improving lives together.

The post holders will work with system partners and be responsible for robust programme management. This will involve:
effective leadership and project management

facilitation and stakeholder management with system providers and commissioners

working alongside designated Senior Responsible Officers and Project Sponsors

creating and driving ownership and accountability and using information to identify problems, create solutions and monitor delivery of change

connecting workstreams across and within the Virtual Health programme, tackling design and delivery issues and risks with energy and passion

These roles are system-wide roles and hosted by Sussex Community NHS Foundation Trust.
